I was in a duel with Kol ShadowSong alone and I randomly had a friend request, it said accept level 0 as your friend, and had no school symbol just a circle.
This sometimes happens when somebody asks to be your friend and then logs off or gets disconnected unexpectedly. When that happens, about all you can do is close the request window.
