I was fighting two covanent initiates and they used quench on me round after round. And the strange thing is they both only had one pip.

A very curious question.

Some monsters and bosses in the game are made to cheat.

Maybe this is your dilemma?

And if they had one pip, are you sure it wasn't a power pip? If it was glowing bright yellow, it could have been.

You will also find some areas where "Weakness is SPAMMED". My suggestion would be to stock up on some "Remove Negative".
