Dalton Prescott, upon coming in contact with him for the first time, says the following in one of the dialogues:
"Following that were thefts of equipment and stores. Then we lost members of our expedition...including our leader...to strange disappearances."
The error can be located in the latter of the prospective dialogue, "to strange disappearances", the correct form would be "two strong disappearances".

Actually, the grammar is correct. Look at it this way: They "lost members of their expedition to strange disappearances."
They added an emphasis on one member, their leader, that was lost to one of those disappearances. They are not referring to a certain number (two, as you interpreted it) of events, but to disappearances in general.
I'm not sure why strange should be changed to strong. People disappearing would seem strange to me!

No it wouldn't, the 'including our leader' is an optional clause offering optional information. So excluding it, the original says:
'Then we lost members of our expedition to strange disappearances.'
Rather than your suggestion:
'Then we lost members of our expedition two strong disappearances.'

It is correct as it is written in the game. How you are suggesting would dramatically change the meaning of the sentence. Dalton Prescott is saying that the members are being lost due to strange disappearances. Not the fact that there were 2 separate events. (I also don't know what a strong disappearance is compared to say a weak one...) Hope that helps clarify!
